Community Support with Delegate Support Services

As a person-centered support service, we strive to provide tailored and individualised community support services and self-care services to enhance the overall well-being and independence of our clients.

Our Community Support services aim to connect our clients with their local communities and provide them with opportunities to engage in social, recreational, and educational activities.

We work with our clients to identify their interests and preferences and support them in finding suitable activities and events. Our community support services also include assistance with transportation, accessing community resources, and support with daily living tasks.

In addition to our community support services, we also offer self-care services that focus on promoting physical, emotional, and mental well-being. Our services are staffed by trained professionals who are committed to providing compassionate care to our clients. We take the time to get to know each individual and their unique needs, so that we can provide the highest level of care possible.

We work with our clients to develop self-care plans that align with their goals and needs, and provide ongoing support and guidance to help them achieve their desired outcomes.

At DSS, we prioritise building meaningful relationships with our clients based on trust, respect, and empathy. We believe that by empowering our clients to make informed decisions and providing them with the necessary support, we can help them achieve greater independence, fulfillment, and quality of life.

SOCIAL & COMMUNITY PARTICIPATION
Support for people to engage in their community, attend social engagements or take part in recreational activities.

Assistance with Social and Community Participation (Support Category 04) is a funded support category that you may find in your NDIS plan, under Core Supports.

This funding also covers support for activities that are either centre-based or out in the community (e.g. to attend a craft/cooking group).

Under Assistance with Social and Community Participation, the NDIS may fund a support worker to support you to:

  • Attend personal development courses
  • Join a social group
  • Take an outing or a holiday
  • Go on a holiday camp
  • Visit your local library
  • See a movie or a concert
  • Go to community events, and/or group activities

DAILY ACTIVITIES
Assistance with Daily Life (Support Category 01) is a funded support category that you may find in your NDIS plan, under Core Supports.

DSS can assistant you to develop the skills needed to improve hygiene, get ready for the day, manage your life, and travel independently, we can assist you to learn how to manage:

  • Showering, dressing, and other personal care tasks
  • Household tasks including cleaning and cooking
  • Home & Yard Maintenance
  • Linen Services
  • Life skills including budgeting and meal planning
  • Travel and transport, including using public transport
  • STA (Short Term Accommodation)
